SIL/SDA/STA & Respite Accommodation

We have properties across South Australia, and we work closely with Participants, their family and Stakeholders to ensure we matched you with the most appropriate place. From the location, the type of housing/building and your protentional future housemates. 

Supported Independent Living for participants | NDIS

Specialist Disability Accommodation explained | NDIS

Short Term Accommodation or
Respite- short time away from your usual home, providing family and the Participant a break. It covers the cost of your care in another place for up to 14 days at a time. It is a flexible short stay with other people, or by yourself, or can be done are your own home. This can assist to prevent Carer's burnout while allowing the Participant try new things.

 

Assessment Recommendation Therapy or Training

We have a range of Qualified Therapists (Occupational Therapists, Counsellors, and Social Workers) to assist Participants & their care team.  From Interventional Care Plan, Training & Education, and Personalize Assessments. 

 

Daily Assistance & Personal Care

From getting out of bed, to personal care, meal preparations, and all in between. We are here to ensure we linked you to appropriate and experience Support Workers to best assist you. We have a range of Support Workers, from Registered/Enrolled Nurses to those with who completed/working towards obtaining their Certificate III in Individual Support. 

 

Finding & Keeping a Job

We have met remarkable people with so many skills to offer. Yet we see the injustice of them not being able to obtain employment due to their disability. We work closely and collaborating with local restaurants, farmers, shops, animal sanctuary, animation studios...just to name a few! To ensure we provide you the opportunity to enhance your employment prospects and career sustainability.  

 

Capacity Building 

Focusing on developing and strengthening your skills and interests. Capacity building involves helping individual with disabilities develop further independence, enabling one to achieve their personal goals an participate more fully within the community.

 

Psychosocial Recovery Coach

With our Recovery Coaches we ensure they hold a minimum of a Diploma level of qualification within the industry. Ranging from Graduate Diploma of Counselling, Mental Health, Enrolled Nursing, Community Services, and Psychology. Recovery Coaches will spend time with you and other relevant people who are import to you. Their job is to get to know, and understand your needs. They are there to help you find out about different service and support, to see how they can help you.

 

Specialist Support Coordination

Coordination puts you at the centre when it comes to engaging with relevant support and services. Which assist to ensure you are accessing and receiving what you are needing and wanting from your formal support.

 

Interpretation/Translating

We are so privilege to have such a multi diverse team, with that we are able to offer on-going to ADHOC translating & interpretation services. Currently we have,

(French, Swahili, Kirundi, Kinyarwanda, Portuguese, Polish, Afrikaans, Mandarin, Arabic & Dutch)

ACTIVITIES OF DAILY LIVING

ADL's are planned around the wishes and capabilities of the participants.

Assistance with ADL's promote participants to remain as active and independent as possible, while receiving the support that they need.

We focus on enhancing the participants ability to progressively learn new tasks, resulting in a sense of well being and achievement.

Groups and individuals  are taught life skills that prepare them for community and and/or shared accommodation living e.g. effective communication,  cooking for a group, problem solving, managing a household and conflict resolution.

 

 

LIFE STAGE TRANSITION

Life stage transition can be daunting and confronting.  We ensure that participants are fully supported when they need to make major changes in their lives e.g. leaving school, getting a job, coping after the death of a partner, learning to live with a new disability  Uptogether provide the following:

  • Assistance with life stage transitions, including mentoring, peer support, individual skill development, and decision making to support people to make transitions on their terms and in a way that meets their goals and aspirations.
  • Assistance to access and maintain employment or higher education.
  • Assistance and support with preparation for school leavers to find employment in either the supported or open job market, and support them in their transition through school towards further education.
